Shane Frost has worked in the community services industry for over 20 years. With a background in youth and disability, Shane has worked in various roles across the service sectors, including in multiple community access and living skills programs supporting people to build capacity to live independently in the community. Following these roles, he continued working with young people in mental health programs, specialising in complex mental health programs that delivered system reform and sector capacity projects, as well as supporting individuals with mental illness to re-engage with support systems.
This work lead into working with complex cases of homelessness, or those at risk of homelessness, that require a multi-agency response. Shane is currently employed by Encircle Community Services to deliver the Service Integration Initiative (SII) in the Moreton Bay region. The SII works to help strengthen systemic structures through regional networks and leadership groups.

Breakout 1: Cross-sector partnerships: Shared systems of care to address homelessness: Cross-sectoral partnerships are fundamental in addressing homelessness. This discussion will explore how partnerships function not simply as referral mechanisms, but as shared systems of care that provide continuity that is achieved and sustained through the recognition that housing, health and social inclusion are fundamentally interconnected. This session will encourage discussion around the role of trust, continuity, and co-location in enabling early intervention, sustaining tenancies, and supporting people to navigate fragmented systems. Join us for this breakout discussion to share insights and explore what works and what doesn’t and what we can learn from each other to build sustainable change.
